Insurance
Clean Slate Video equipment will not be released to client unless proper insurance has been arranged for in advance. Clean Slate will be provided with a certificate of insurance naming Clean Slate as "additional insured" and "loss payee" for an amount equal to or greater than the replacement value of rented equipment. If Clean Slate Video provides transportation for rental, the producer shall also provide "hired vehicle" insurance. Deductibles arising from any insurance claim will be paid by producer.
Indemnity
Clean Slate Video, when packaging crews for producers, is acting as an agent for the producer. The producer will hold Clean Slate Video harmless in the event that any claims arise from the production regardless of fault. This shall include, but not be limited to, "operator error", faulty tape stock, injury to crew members or others due to crew members' negligence or unsafe working conditions.
Payment
Until credit has been established, payment for equipment rentals and crew services will be made at the end of shooting (if not sooner). Failure to do so will result in Clean Slate retaining possession of shot media until payment has been made. When credit has been established, terms will be Net: 30 days. Any balance due after that period will be subject to a monthly interest rate of 1.5% per month.
Cancellation Policy
If a booked production cancels between 48 and 72 hours before scheduled production start, a cancellation fee of 25% of projected invoice will be billed. If cancellation occurs 24 to 48 hours prior, a fee of 50% will be charged. And if there is less than 24 hours notice prior to a cancellation, the producer will be liable for the entire projected invoice amount.
